Performs coordination of patient placement and transfer services to residential health care and rehabilitation facilities.
_Education:_ Bachelor's Degree in Social Work or Social Services from an accredited University with one year's experience in a medical setting required or a Bachelor's Degree in a related field from an accredited University with three years experience in a medical setting required.
_License/Certification:_ New York State Department of Health certification for completion of the New York State Department of Health Screen preferred. Must obtain certification in NY within 6 months in the position.

_Skills:_ Knowledge of regulatory requirements governing transfer of patients from the acute to the post-acute inpatient level of care.Estimated Salary: $20 to $28 per hour based on qualifications.
